Dallas Family Medicine is committed to serving our community as providers of healthcare and medical education.
Continuing in a tradition established more than 50 years ago, we strive to heal the whole family with care and compassion, from the infant to the elderly.
In our innovative education program, we endeavor to share our passion for medical education through current relevant teaching you can trust. We are proud to serve you - this is our heritage of caring.
DFM is pleased to welcome Julia Thomas, FNP!
Julia grew up in Polk County and is happy to return as a Nurse Practitioner to care for the community she loves. Her education and experience have included nursing in the Emergency Department of Salem Hospital, volunteering as a relief worker in Romania, helping to establish the Polk Community Free Clinic, and this summer she will complete her doctoral studies in Nursing Practice at OHSU.
We are so glad to have her join us and know you and your family will enjoy getting to know her as well.

Please, take the time to learn about, investigate and know the primary care team you choose.
Choosing a primary care provider is one of the most important decisions you will make in your life. Your relationship with your healthcare team of physicians, PA's, and patient care support staff is an integral component of your well-being. Our patients know this.
We welcome your questions and encourage you to visit us and tour our facility.
Dallas Family Medicine's services are extensive and specialized across the lifespan.
Newborn & Pediatric Care
Patient Urgent Care
DFM has been providing primary care services for over 60 years. Over the decades we have seen the health care system go through many changes and challenges, but have found a way to continue to care for our patients.
The Affordable Care Act (aka “Obamacare”) is complex and represents the greatest change to the healthcare system since the adoption of Medicare, but DFM is committed to serving our community and we will still be here to care for your family long after the Affordable Care Act fades from the news.
Many aspects of the new law are controversial and there is a lot of debate about how we will all be effected. For our purposes, we will try to focus on what you have to do and how DFM can help.
Part of DFM's decades-long commitment to primary care has included offering lab services. Advances in medicine and the changing needs of our patients require us to adapt, while holding to our commitment to quality and our local community. To that end, we are pleased to announce our relationship with West Valley Hospital for laboratory services.
As of 2.17.2014, West Valley Hospital assumed ownership and operation of the lab for patients who choose the convenience of the laboratory located in DFM. You will still see the friendly face of Grant, our current Lab Technician. Bills will come from West Valley Hospital.
As always, you are free to use a lab of your own choosing, but we are glad this service will remain a convenience to our DFM family. We are also very interested in your feedback - if you have questions or comments, please, let us know!
